GAPP In-Home Care Services
Our in-home services under the GAPP program include a wide range of support to assist with daily life and complex medical needs, such as:
- Assistance with daily living activities, including bathing, meals, and light housekeeping.
- Monitoring of vital signs, including blood pressure, pulse, respiration, and temperature.
- Mobility assistance, including ambulation and transfers.
- Skilled nursing care, such as IV therapies, wound care, and tube feedings.
- Management of gastrointestinal disorders.

The Process
Step 1: The Initial Consultation
It all starts with a simple phone call. In this first conversation, we’ll listen carefully to understand your situation and gather preliminary information.
This allows us to get a clear picture of your needs right from the start. We will then schedule a convenient time for an in-home meeting with you and your family.
Step 2: Crafting Your Personalized Care Plan
During our in-home meeting, we conduct a more detailed assessment to fully understand the specific requirements of the client. Based on this, we develop a customized care plan that outlines a recommended course of action.
We will review this plan with you and your family to ensure everyone is comfortable and in agreement before moving forward.
Step 3: Matching You with the Right Caregiver
Once the care plan is approved, we begin the important process of matching you with the ideal caregiver from our team. We use a careful selection process to find a professional whose skills, personality, and experience are the perfect fit for your family.
Our caregivers maintain detailed notes, and a supervisory nurse conducts regular, unannounced visits to ensure the highest quality of care.
If for any reason you feel the match isn’t right, we are always happy to find another caregiver.
